Welcome to the Hanover Family Health Team


Established in 2011, the Hanover Family Health Team (HFHT) is a group of specialized health care professionals working together to coordinate the highest level of patient care possible. Specifically constructed based on the needs in our area and to close gaps in service, our mission is simple – improve the overall health and quality of life for individuals living within our community.

The Hanover Family Health Team is now offering flu shots by appointment only. Please contact us to receive your flu shot today!

The Hanover Medical Clinic also offers flu shot clinics. Please see below for details:

 November 7 - 9:00am to 12:00pm at the Medical Clinic

 November 14 - 2:00pm to 5:00pm at the Medical Clinic

 November 21 - 9:00am to 12:00pm at the Medical Clinic

 November 28 - 2:00pm to 5:00pm at the Medical Clinic

Monday          8:30am – 4:30pm

Tuesday          8:30am – 4:30pm

Wednesday    8:30am – 4:30pm

Thursday        8:30am – 4:30pm

Friday             8:30am – 4:30pm

Saturday        CLOSED

Sunday           CLOSED

The Family Health Team is closed on statutory holidays.

Lung Health Program

The Lung Health Program is delivered by our Nurse Practitioner, Pharmacist/CRE, and Registered Nurse/CRE and supports rostered patients with:

- Asthma 

- COPD (Chronic Obstructive Pulmonary Disease)

- Chronic Lung Infections

- Past or Present Use of Inhalers 

- History of Cigarette Smoking

Memory Clinic

The Memory Clinic is an inter-professional approach to providing early diagnosis and treatment for problems associated with memory loss.  The team members work with your family physician at the Hanover Medical Clinic to provide comprehensive care for patients and family members dealing with dementia and other conditions involving memory loss.  For many of these conditions, early diagnosis and treatment can help to maintain independent living and quality of life for as long as possible.

Ontario EarlyOn Child and Family Centre Hanover

On a monthly basis, healthcare professionals from the FHT join the Hanover EarlyOn Child and Family Centre to present information and provide resources on various topics that are important to families in our community.

Mild Cognitive Impairment Group

Learning the Ropes for Mild Cognitive Impairment is facilitated by our Occupational Therapist and Registered Nurse for this 6 week program. We are excited to offer this innovative memory training program in partnership with Baycrest and as a complement to our Memory Clinic. Baycrest is an academic centre fully affiliated with the University of Toronto.

Prenatal

Prenatal classes are a fun way of connecting with other pregnant women and their partners and learning what to expect during life with your new baby. The Prenatal Program is a 4-week series offered each week for 2 hours. There is no cost to participate in this program.
