Social Worker-Permanent Full-Time

The Hanover Family Health Team is seeking an enthusiastic Social Worker who is both self-motivated and who would also enjoy working collaboratively with our team of physicians and interdisciplinary health professionals to provide high quality patient-centred primary care. This is a permanent full-time position.

Role and Responsibilities:

  • Provide primary mental health screening, assessment and treatment for mild to moderate mental health issues
  • Provide counselling for groups, families and individuals of all ages
  • Assist patients and providers in identifying and connecting with appropriate health, social and community service providers and agencies
  • Assist patients with filling out forms and obtaining social and financial supports
  • Case management, including links to the hospital and community services
  • Develop, update and maintain educational resources for patients and providers
  • Be a strong patient advocate 

Position Requirements:

  • Bachelor’s or Master’s Degree in Social Work
  • Member in good standing with the Ontario College of Social Workers and Social Service Workers and proof of professional liability insurance
  • 3 years community-based experience preferred
  • Knowledge of local and regional community resources related to the provision of mental health and social services
  • Demonstration of excellent clinical judgement, assessment, treatment, case management, program development, teaching, problem solving and decision making skills
  • Proficient computer skills
  • Valid Ontario driver’s license and access to a vehicle

                     Data Quality Specialist (Contract Position)

This position will be of particular interest to individuals pursuing a career in healthcare or computer sciences. The successful applicant will work collaboratively to enhance data quality and standardization in our electronic medical records.

Position Requirements:

• High level of computer proficiency
• Exceptional organizational, communication, time management and problem solving skills
• Exceedingly high degree of accuracy and attention to detail
• Medical terminology knowledge an asset
• Able to work efficiently and independently
• Must adhere to all privacy and confidentiality policies

This 14-week contract position is funded through Canada Summer Jobs and to qualify candidates must be between the ages of 15 and 30.
