The Hanover Family Health Team is currently seeking a Quality Improvement Decision Support-Specialist (QIDSS)

This position is Permanent Full-Time.

Background:
The Brockton and Area Family Health Team, in collaboration with East Elgin, Hanover, Kincardine, Owen Sound and Sauble Family Health Teams (QIDSS Partnership) is recruiting for a full-time Quality Improvement Decision Support Specialist. The QIDSS will work with the senior primary care leadership of the above FHT’s to advance the impact of data and performance measurement in the provision of accessible and comprehensive primary care.

Position Overview:
Reporting to the QIDSS Partnership, the Quality Improvement Decision Support Specialist is responsible for supporting multiple sites in meeting their quality improvement objectives with the use of data to guide clinical decision-making. Key responsibilities include supporting teams in the implementation of data quality initiatives, improving the flow and use of information, developing queries and analytical products that support boards and leaders in their quality improvement goals, and engaging teams in clinical process change. The incumbent will also be expected to participate in broader data harmonization initiatives and champion the use of data and performance analytics within and across Ontario’s family health teams.

Key Areas of Responsibility:
• Support FHT leadership and Quality Improvement Committees in meeting accountability requirements
• Create effective working relationships with multiple teams to assist with the management of data quality, business process improvement, report development and other projects
• Be a subject matter expert in data integrity and lead the analysis of data quality issues, using problem-solving methodologies to recommend corrective and preventative action
• Collaborate closely with multi-disciplinary teams engaged in quality improvement, helping to identify performance measurement information and data quality needs
• Design, test and deploy tools and processes for extracting and managing performance data
• Assist with documentation and sharing of best practices in data management and quality improvement including data collection protocols, searches, program monitoring and evaluation criteria
• Act as a change agent and coach to inter-disciplinary teams on data management and performance measurement
• Work with your peers across the province on standards development and data harmonization priorities
• Support team awareness and adoption of provincial QI initiatives
• Monitor and evaluate clinical information system functionality from a user and decision support perspective
• Be a local expert user for multiple EMR vendors and databases

Key Qualifications:
• A university degree in a related discipline (Statistics, Health Sciences, Epidemiology or Information Management) or equivalent education and experience
• Demonstrated expertise in data quality and clinical information management best practices
• Currency in applicable privacy and confidentiality policies and laws regarding the collection and use of health information
• Knowledge of primary care and demonstrated interest in the use of data for quality improvement
• Strong applied knowledge of one or more EMR/EHR systems currently in use in Ontario
• Proficiency in programming languages and reporting writing tools (SQL)
• Strong applied knowledge of Microsoft Office Suite (Excel, Word, Power Point, Access)
• Sound knowledge of performance improvement techniques and practices used in healthcare settings, process redesign and system implementation experience
• Highly effective communication, presentation and interpersonal skills and a proven ability to explain concepts to end users with varying degrees of comfort with technology
• Ability to handle a variety of concurrent assignments and to work independently
• Must have own vehicle and a valid Ontario driver's license
