Quality Improvement

We always appreciate and welcome input from our community and from our patients. In this regard, and in order to monitor and to strive to improve the quality of our services, from time to time we will ask patients how they found their experience when using our services here at the Hanover Family Health Team. We appreciate everyone who participates by filling out the surveys and/or sharing their comments with us. Since you took the time to fill out these questionnaires, we thought it only fair that we share the results with you. These results were from the questionnaires that were filled out during our random surveys which were distributed in 2019-2020.

The Hanover Family Health Team continues to demonstrate exceptional patient-centred care which is evidenced by the following results:

• In our in-house patient surveys, 100% of patients reported that when they see an interprofessional healthcare provider, they or someone else in the office always/often give them an opportunity to ask questions about their treatment,

• 100% of patients reported that when they see an interprofessional healthcare provider, they or someone else in the office always/often involve them as much as they want to be in decisions about their care and treatment,

• 100% of patients reported that when they see an interprofessional healthcare provider, they or someone else in the office always/often spend enough time with them,

• 100% of patients reported that when they need an appointment for an urgent matter, they were seen by an interprofessional healthcare provider within 3 days and of those, 91% were seen same day/next day,

• 100% of patients reported that they were pleased/very pleased with the length of time it took to see an interprofessional healthcare provider, from the time they called for an appointment to the time that they were actually seen, and

• 100% of patients reported that their overall experience with our reception staff was excellent.
