1. How Do I Enroll with a Family Physician?

Health Care Connect is designed to help you find a family health care provider. You will also find information and resources to help you access additional health care services in your community.

2. Is my Physician a part of the Hanover Family Health Team (HFHT)?
All of the physicians working at the Hanover Medical Associates are affiliated with the HFHT. We work together to connect patients with necessary resources and strive to provide the best possible care to our patients.

3. How can I see one of the professionals of the HFHT?
To see your family physician simply call the Hanover Medical Associates to book an appointment. To see one of the HFHTs allied health professionals, talk to your family physician about a referral. Once the referral has been made, you will be contacted by a member of the HFHT to arrange an appointment. You can also self-refer by calling the HFHT directly and booking an appointment. You must be a registered patient of the Hanover Medical Associates in order to utilize the services at the HFHT.

4. How will the HFHT benefit me?
The HFHT allows patients to access specialized health care services in a timely manner. Our 'no barriers' approach to healthcare means support is always available if you need it. Our services focus primarily on disease prevention, health promotion and disease management.

5. How much will this cost me?
There is no cost to you for any of the services. All the programs and services offered by the HFHT are funded by the Ontario Ministry of Health and Long-Term Care.

6. How do I cancel an appointment?
Please contact the Hanover Family Health Team office at 519-506-4348 as soon as possible and preferably at least 24 hours prior to your scheduled appointment to cancel.

7. Who should I call for medical assistance when the Hanover Family Health Team is closed?
In event of an emergency, call 911 or go to your nearest hospital emergency room.

If you have a general health question requiring medical advice, call:
Telehealth Ontario at 1-866-797-0000.

A call to Telehealth Ontario will give you confidential advice about any health-related concerns such as:

  • symptoms that could require medical attention
  • illness or injury
  • chronic illnesses
  • nutrition and healthy lifestyles
  • teen health and lifestyle issues

A call to Telehealth Ontario does not replace 911.

911 is ALWAYS THE FIRST NUMBER TO CALL IN EMERGENCY SITUATIONS.

8. What Does a Nurse Practitioner do?

Nurse Practitioners provide care to patients of all ages with a focus on preventing disease, promoting good health and treating illness. They are part of your health care team. The Nurse Practitioner plays an important role in helping you to maintain a healthy, active lifestyle, and to understand and/or treat your illness. They work with our community to promote good health and prevent disease. As a member of your health care team, our Nurse Practitioner offers a variety of health care services including the ability to:

  • diagnose and treat illness and/or injuries
  • perform physical check-ups
  • order and interpret diagnostic tests
  • write prescriptions
  • provide counselling and education
  • provide supportive care through illness
  • provide treatments and/or procedures
  • make referrals to family physicians, specialists and other health professionals

9. How do I sign up for one of the programs?
